This questionnaire will help the S-CAP Teaching Team better prepare for conducting a workshop in your area. Please fill out each question thoughtfully and submit to your S-CAP Liaison at least
two weeks prior to your scheduled workshop.

What is your local definition of “livestock”? (e.g., does it include horses? poultry? pets?)

Are there any nuclear reactors nearby? If so, where?

What are some recent disasters that have occurred in your state? Was agriculture affected? If so, how? How would you rate the local jurisdiction(s)’ response to the incident?

Do the communities attending the workshop have active Local Emergency Planning Committees (LEPCs)? If so, are they strictly hazardous materials planning groups, or is their scope broader (e.g., all-hazards)?

What type of local governance is in place? (e.g., board of commissioners, mayor, judge/executive) Who is the Chief Elected Official at the local level? What is the term for a local jurisdiction in your state (e.g., county, parish, borough)?

Does your state have Community Organizations Active in Disaster (COADs)? If so, what resources do they have?

Does your state have a State Agricultural Response Team (SART)? What about Community Agricultural Response Teams (CARTs)? If so, what resources do they have?
